Goalkeeper Farouk Ben Mustapha was subbed on for the shootout and proved to be the hero of the hour saving Leeds United striker Caleb Ekuban's spot-kick for Ghana.
Tunisia will now face the competition's surprise package Madagascar in their quarter-final on Thursday.
After Taha Yassine Khenissi had given them the lead, The Eagles of Carthage had been moments away from winning in normal time only for substitute Rami Bedoui to score a calamitous own goal in injury-time with his first touch. They have now drawn all four of their games at the tournament.
For Ghana, this shootout defeat marked the first time since 2008 that the Black Stars have failed to make it through to the last four in the competition.
Elephants trample Malian hopes

The forward, currently linked with a summer move to Arsenal, scored The Elephants' matchwinner - with their less-experienced opponents Mali left to rue a string of missed chances.
Ivory Coast will now play in-form Algeria in their quarter-final on Thursday.
